The Community Health Worker Team & The Needs It Addresses

The Community Health Worker team is an integral part of the Community Based Case Management approach here at Sadler.

Main goals of the team are to utilize the results of the PRAPARE tool to address social determinants of health, which impact patients’ ability to access healthcare, and to help close those gaps that are stopping patients from getting the care that they need.

Barriers such as food insecurity, housing insecurity, lack of transportation, loss of employment or health insurance, lack of support systems, or language issues are top reasons why appointments are missed or not scheduled.

We continue to seek out local events to see how we can establish a strong Sadler presence, participate in vaccines clinics and promote our mobile van services. We are here to advocate for our patient’s individual and community needs and close those gaps in care. We are strongly focused on addressing needs that have been identified by many of the providers, behavioral health specialists, case management and others who have referred patients with socio-economic needs to CHW for assistance.
